Forms & Downloads

Complete these forms in advance to save time at check-in. Fillable forms can be completed, signed, and sent to us online; the others can be downloaded, printed, and returned by email or mail.

How Fillable PDF Forms Work

Some forms on this site are fillable directly on your computer or phone. Selecting New Patient Packet or Authorization to Disclose Protected Health Information opens our secure PDF-filler tool, where you can fill, sign, and send your documents straight to our practice, or print or email yourself a copy. A downloadable version is also available; a few fields on that version must be completed by hand after printing. Completed and scanned forms can be emailed to [email protected].

Your Appointment

Here's what to expect, what to bring, and a few notes for contact lens wearers and cancellations.
What to Expect at Your Appointment

Your appointment is a comprehensive evaluation and will take at least 2 hours. Your visit may take longer if you have additional testing or complex eye problems.

We recommend completing the new patient packet in advance and returning it to us by mail, email, or in person before your appointment.

When you arrive, please bring:

Insurance cardsAll medical insurance cards.
Photo IDDriver's license, state ID, passport, or college ID.
List of medications
Payment methodFor co-pays and non-covered out-of-pocket fees.
A parent or legal guardianRequired if the patient is a minor.
Completed formsMedical History & Patient Information forms (new patients only).
A driverIf you're not comfortable driving with dilated eyes.
Cataract and LASIK Evaluations

In addition to your comprehensive appointment, testing specific to your evaluation will be performed. You'll also have an opportunity to review consent forms and, if you choose, schedule your procedure the same day. We recommend planning on being in the office for approximately 3 hours.

Contact Lens Wearers
Soft contact lensesRemove 3–5 days prior to your appointment.
Hard contact lensesRemove 7 days for every 10 years worn. For example, a 30-year wearer should remove lenses about 21 days prior.
Canceling an Appointment

We recognize that illness and other unplanned circumstances arise. If you cannot keep your appointment, please give our office at least 24 hours' notice so we can offer the slot to another patient.
